Alliance Health is seeking a Care Manager for the Care Management for High-Risk Pregnancy program in Fayetteville, NC. This full-time hybrid role supports early identification, assessment, care planning, provider coordination, education, and postpartum transition planning for eligible high-risk pregnant members.

Responsibilities include outreach, enrollment support, development of individualized care plans, coordination with OB and community partners, and documentation in the care management

Qualifications

• Master’s degree in Human Services or related field with at least two years of post-graduate care management experience.
• Must be licensed or provisionally licensed in North Carolina as an LCSW, LMFT, LCAS, LCMHC, or LPA.
• Or a nursing degree with NC RN licensure and three years of post-degree experience.

Responsabilités

• Provide comprehensive, member-centered care management for high-risk pregnancy.
• Identify and address barriers to prenatal care and social needs.
• Coordinate with OB providers and community partners.
• Document outreach and care plans in the care management platform.
• Travel within Alliance communities as needed.
