Job summary

The Care Management for High-Risk Pregnancy (CMHRP) Care Manager position at Alliance Health provides comprehensive, member-centered care management for eligible high-risk pregnant members. This role supports outreach, assessment, care planning, and coordination with providers to ensure a whole-person approach.

Reporting to the Care Management Team, the role requires NC licensure (LCSW/LMFT/LCAS/LCMHC/LPA or RN with NC license) and a master’s degree in Human Services.

Qualifications

• Master’s degree in Human Services or related field.
• Two years of full-time post-graduate care management or related experience.
• Licensed in NC as LCSW/LMFT/LCAS/LCMHC/LPA or RN with NC license.
• Preferred: experience in care management or Medicaid/behavioral health.

Responsibilities

• Outreach to and support pregnant members in CMHRP during pregnancy and postpartum.
• Develop and maintain individualized care plans with providers and supports.
• Coordinate with OB, behavioral health, hospitals, and community partners.
• Educate members on prenatal care engagement, barriers, and resources.
• Identify barriers and escalate urgent concerns per approved workflows.
• Document outreach, assessments, care plans and referrals timely.
