Job summary

Evergreen Nephrology seeks an Implementation Manager for Evergreen Connected Care to drive onboarding and ongoing relationships with partner practices. You will own the end-to-end onboarding sequence, manage timelines, and coordinate with clinical and program leaders to scale the execution as the program grows.

This fully remote role requires exceptional communication, multi-stakeholder coordination, and occasional travel to practice sites. Compensation ranges from $100,000 to $110,000 annually.

Qualifications

• 5+ years of experience in implementation, program or project management, client success, or practice/provider operations.
• Experience running multi-stakeholder implementations to a defined timeline with external customers or partners.
• Excellent written and verbal communication skills with ability to lead meetings with clinicians and executives.

Responsibilities

• Lead onboarding and implementation for Evergreen Connected Care across partner practices.
• Maintain timelines, run meetings, and ensure deliverables and owners are clearly defined.
• Coordinate with Finance, Legal, Compliance, Data, People Ops and Market Ops to unblock work.
• Serve as primary day-to-day contact for partner leadership and clinical leads.
• Track deadlines and drive items to closure, escalation only when needed.
