Responsibilities

• Anchor the remote care team through NRP Certification and Wound Care cases from intake to follow-up at Medford, OR.
• Verify two patient identifiers before every draw, scan, and medication pass.
• Draw labs, run point-of-care testing, and flag abnormal panels to the supervising clinician.
• Trace medication discrepancies back to the source order, then close the gap with pharmacy.
• Counsel patients on discharge meds to reduce readmissions.
• Round on Medford, OR long-stay patients to monitor for issues.
• Discuss prognosis, code status, and goals of care with families.
• Document refusals, allergies, and advance directives for team access.
