Job summary

North Sound ACH in Washington state seeks a Health Networks Manager to serve as the external anchor for the Care Coordination Hub, building and stewarding the HRSN network and partnership strategy to address housing instability, food insecurity, transportation, and other social needs.

You will collaborate with the Community HUB Manager to align external referrals, train frontline staff, and lead regional partnerships with health systems, MCOs, AAAs, and housing and transportation providers,

Qualifications

• Five years of experience or achievements that encompasses oversight of programs related to health and health improvement.
• Experience managing social determinants of health (SDOH) programs or state Medicaid waiver initiatives, and working within integrated care models, managed care, or community health networks.
• Proven success navigating systems for Medicaid, Medicare, or historically marginalized populations, with knowledge of regional social safety nets and public benefits.
• Technical literacy with social referral platforms and their integration with client management systems.

Responsibilities

• Inform strategy by providing external-network intelligence to Hub planning.
• Align referral pathways with Hub workflows to ensure seamless navigation.
• Equip staff with content on HRSN partners, resources, and referral pathways for training.
• Support complex cases with network and resource expertise for interdisciplinary reviews.
• Cultivate and steward external HRSN networks with formal partnerships and referrals.
• Negotiate service expectations and workflows with housing, food, and transportation providers.
• Assess regional capacity and identify service gaps.
• Represent the Hub at coalitions, task forces, and health equity forums.
