Job summary

Forward Leading IPA is seeking an SCN Enhanced Care Manager to support high-risk and specialty populations through the Social Care Network. Working in a hybrid model with periodic in-person meetings, you will perform HRSN screenings, eligibility assessments, care planning, referral management, and outcome tracking.

You will build relationships with members, families, and providers, coordinate with OB/GYN practices, WIC offices, FQHCs, and community partners, and participate in multidisciplinary

Qualifications

• Bachelor’s degree in social work, Human Services, Public Health, Nursing, Psychology, or a related field.
• Minimum of two years of experience in care management, case management, care coordination, community health, or related area.
• Experience working with vulnerable, high-need, or underserved populations.
• Knowledge of community resources, healthcare and social service systems.

Responsibilities

• Manage a dedicated caseload of high-risk and specialty populations.
• Complete screenings and assessments to identify health-related social needs and eligibility.
• Develop, implement, and update individualized care plans.
• Connect members to providers, programs, and community resources.
• Monitor progress, referral outcomes, and service engagement.
• Provide ongoing support, education, and follow-up to navigate services.
