Job summary

BlueCross BlueShield of Tennessee is seeking a Health Navigator to support the BlueCare Plus Clinical Support team. You will act as a primary contact for members, guiding them through healthcare options, answering questions, and completing health assessments with compassionate, clear communication.

The role emphasizes remote work, strong written and verbal skills, and the ability to work independently while supporting a wide range of member needs and coordinating with clinical staff.

Qualifications

• Associates Degree in education, communication, or health related field or equivalent work experience.
• At least 2 years in a customer service support role.
• Knowledge of medical terminology and ability to work remotely.

Responsibilities

• Conduct educational telephone calls advising members of available benefits, services and programs; completes health needs assessment and refers to population health management programs as appropriate.
• Reach out to members with identified gaps in care; encourage and assist with appointments and provider referrals.
• Manage system work queues; screen members for eligibility, prior activities, claims, inquiries and history; coordinate with clinical team for outreach.
