Job summary

BlueCross BlueShield of Tennessee is seeking a CHOICES Care Coordinator to positively impact members’ lives through home visits and coordinated support services. The role emphasizes problem-solving, time management, and a passion for improving health outcomes in diverse communities.

Qualifications

3 years of experience in a clinical setting.
Registered nurse with an active license in Tennessee or license in their residence state if that state participates in the Nurse Licensure Compact Act; or Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
Exceptional customer service skills
Must live within Shelby County, TN
Available for 8:00am – 5:00pm EST (no on call); potential compressed work week after 1 year with management approval

Responsibilities

Partner with members and families to identify supports and services to meet personal health goals.
Collaborate with clinical and social support teams to address member needs.
Conduct thorough face-to-face visits and assess physical, behavioral, functional, psycho-social, financial, and living needs.
Authorize appropriate home and community services and ensure needs are met.
Maintain valid Driver’s License and TB skin test if field-based.
Position requires 24 months in role before posting to other internal roles.
