Job summary

Ascend Medical is seeking a remote Health Concierge with a primary focus on senior care services, including support for ALF patients. The role supports inbound inquiries via digital campaigns and referrals, guiding families from first contact to appointment scheduling and portal setup.

Qualifications

• Clinical background required; CMA certification or higher strongly required.
• Experience working with senior populations, assisted living, or primary care is highly valued.
• Familiarity with insurance verification and healthcare systems.

Responsibilities

• Serve as the first point of contact for senior care patients and their families
• Schedule new patient appointments via phone and future digital channels
• Verify insurance benefits and clearly explain patient cost responsibility
• Document patient information accurately in the EHR and Salesforce
• Communicate effectively with clinical teams regarding patient needs
• Educate patients and families on Ascend services and how to access care
• Provide compassionate, high-touch service tailored to senior populations
• Support scheduling needs for Assisted Living Facilities, including later-hour coverage when needed
