Qualifications

• Bachelor Degree OR 1 year of college and 3 years health, social service or community experience, OR 4 years' health, social services or community experience
• Knowledge of medical terminology
• Experience coordinating member related needs, providing assistance to members, and analyzing member needs
• PC proficiency including Microsoft Office applications
• Customer service skills
• Verbal and written communications skills including developing written correspondence to members and to other department personnel and coaching skills, including motivational interviewing

Responsibilities

• Contact members and community partners to support healthcare initiatives and activities
• Assist in scheduling care manager and member appointments and follow-ups
• Support clinicians in the physical and behavioral health care management department with provider and member activities
• Support relationships with community partners and community-based organizations
• Contribute to the design of community health outreach programs
• May require field-based work in the Northwest & South Chicagoland area
