Job summary

SCAN is seeking a clinical pharmacist to join a multidisciplinary care team, providing evidence-based, cost-effective drug therapy for seniors. You will perform comprehensive medication reviews for high-risk patients, collaborate with NP/PCP, and lead deprescribing and adherence initiatives.

You will communicate recommendations, educate staff, and help improve access and affordability of medications while supporting formulary optimization and patient outcomes.

Qualifications

• Active Pharmacist License required (CA)
• Experience with chronic disease states (diabetes, heart failure, hypertension, asthma, COPD)
• Completion of PGY1/PGY2 residency or equivalent clinical practice preferred

Responsibilities

• Complete comprehensive medication reviews for high-risk, complex patients and suggest changes to NP/PCP
• Evaluate therapeutic effectiveness and optimize treatment while minimizing medication burden
• Educate colleagues and lead pharmacy initiatives on deprescribing and medication adherence
• Identify barriers to access, affordability, and adherence and coordinate with providers and pharmacies
• Support formulary optimization and multidisciplinary case reviews
