Pine Park Health Management Inc. in Milpitas, California, is seeking a full‑time Nurse Practitioner to deliver care in senior living communities within the San Francisco Peninsula market (Palo Alto, Redwood City, San Mateo).

You will join a values‑driven, multidisciplinary team focused on a holistic, patient‑centered model of care. The role emphasizes a smaller patient panel of complex cases, collaboration with navigators and a flexible hybrid schedule that supports in‑person and remote work.

Qualifications

• Must be a licensed Nurse Practitioner with geriatrics experience.
• Able to thrive in a fast-paced, changing environment.
• Willing to provide onsite clinical across the San Francisco Peninsula region.
• Desire to work with complex care patients and support holistic care delivery.
• Strong orientation towards collaboration with a multidisciplinary team.
• Excellent written and verbal communication skills.
• Experience with tech-enabled care delivery tools (project management, checklists, remote monitoring).
• Willingness to travel between partner communities as needed.

Responsibilities

• Provide onsite primary care to older adults living in Senior Living Communities.
• Develop and execute detailed patient-centered care plans for all primary care patients.
• Collaborate with Medical Director to develop policies affecting daily practice and guidelines.
• Provide monthly Chronic Care Management visits and review CCM documentation.
• Lead regular huddles and provide leadership to the clinical team.
• Coordinate with senior living management and care teams to improve Pine Park care plans and patient outcomes.
