Job summary

Pine Park Health is seeking a Registered Nurse for a fully remote role serving residents in California, Nevada, and Arizona from a home office. You will work with PCPs, Medical Directors, and Medical Assistants to deliver coordinated primary care to seniors in living communities.

Qualifications

• Current unrestricted RN license (California)
• 2+ years of clinical experience in geriatrics or related fields
• Strong communication and ownership in fast-paced environments

Responsibilities

• Collaborate with PCPs, Medical Directors, and Medical Assistants to manage a large patient panel
• Triage and route inquiries via calls, faxes, email, and messages; escalate to PCPs when needed
• Support LVN/LPNs and enhance team efficiency
• Prepare charts and educate patients on care plan adjustments
• Oversee post-visit processes: orders, referrals, and meds
• Participate in daily huddles and provide guidance to the care team
• Conduct virtual assessments and develop personalized care plans
• Serve as a communication link among patients, PCPs, specialists and other care professionals
• Promote preventive care and patient self-management strategies
• Engage in crisis intervention and palliative care discussions
• Contribute to quality improvement initiatives and geriatric research
