Qualifications

Responsibilities

• Participate in interdisciplinary care planning and care coordination.
• Assess participants using clinical judgment and advanced nursing skills.
• Document and interpret diagnostic results and progress notes.
• Coordinate with IDT and manage care plans and visits.
