Job summary

Obran Health is seeking an experienced RN Clinical Coordinator to join our team in a fully remote, full-time W2 capacity. You will provide critical clinical coordination and operational support across our California-based home health agencies.

Partner with DPCS and clinicians to ensure documentation accuracy, support field staff, and maintain high-quality patient care. Must hold an active California RN license and have 1–2 years home health experience; prior supervisory experience is required.

Qualifications

• Active, unrestricted California RN license.
• 1-2 years of hands-on home health RN experience.
• Minimum of 1 year of supervisory experience in home healthcare or a similar field.
• Proficiency with Kinnser/WellSky EMR strongly preferred.
• Remote capability with strong digital communication skills.

Responsibilities

• Provide care coordination and clinical oversight for assigned patient caseloads across both agencies.
• Review, audit, and sign clinical documentation including SOC, Recertifications, ROC, discharges, and external orders.
• Serve as point of contact for incoming clinical inquiries from field staff and patients; escalate to DPCS as needed.
• Collaborate with physicians and providers to maintain continuity of care.
• Analyze documentation/workflows to identify quality improvement opportunities and ensure policy compliance.
• Support remote onboarding, training, and clinical guidance for field clinicians.
