Pine Park Health, Inc. is seeking a Remote Registered Nurse with an active California RN license to support geriatric patients across CA, NV, and AZ from a home office. You will collaborate with PCPs, MAs, and care teams to deliver high-quality primary care in a tech-enabled setting.

The role emphasizes remote care coordination, patient education, and crisis/palliative discussions while advancing geriatric care outcomes. Prior remote experience and strong communication are essential.

Competenze

• A current unrestricted RN license with multi-state eligibility and minimum 2 years of clinical experience, preferably in geriatrics.
• Proven success in remote roles using technology and digital communications.
• Strong accountability, clear communication, and ability to thrive in a fast-paced setting.
• Experience with geriatric patients in senior living or skilled nursing environments.

Mansioni

• Collaborate with PCPs to support a large patient panel.
• Triage calls, emails, faxes, and messages; escalate to PCP when needed.
• Support LVN/LPN teammates and improve team efficiency.
• Prepare comprehensive chart reviews and educate on care plans.
• Oversee post-appointment orders and referrals.
• Participate in daily huddles and guide the care team.
• Conduct virtual assessments and develop personalized plans.
