Job summary

Pine Park Health Management Inc is seeking a Remote Registered Nurse (CA RN License Required) to deliver high-quality primary care to residents of senior living communities from a home office. This fully remote role requires an active California RN license and offers collaboration with PCPs, Medical Directors, and Medical Assistants across California, Nevada, and Arizona.

Qualifications

• Active, unrestricted California RN license.
• Minimum 2 years of clinical experience, preferably geriatrics.
• Experience in remote roles and digital communication platforms.
• Strong accountability, communication and teamwork in fast-paced environments.
• Experience with geriatric patients in senior living or skilled facilities.

Responsibilities

• Collaborate with PCPs to manage a large patient panel.
• Triage calls, faxes, emails, and messages; escalate when needed.
• Support LVN/LPNs within the team to improve efficiency.
• Prepare charts and educate patients on care plan adjustments.
• Oversee post-appointment orders, referrals, and medications.
• Participate in daily huddles with PCPs and MAs."
• Conduct virtual patient assessments and develop care plans.
• Serve as a communication link among patients, PCPs, specialists, and other clinicians.
• Promote preventive care and self-management per the care plan.
• Engage in crisis intervention and palliative care discussions.
• Contribute to quality improvement and geriatric care research.
