Total Life is a telehealth company focused on comprehensive geriatric care for adults 65+. You will deliver initial longevity assessments and ongoing follow-ups to older adults via telehealth, under the guidance of our geriatric CMO.

Training and clinical support are provided, with a focus on Medicare billing and high-quality patient care. The role emphasizes cognitive screening, medication review, and management of chronic conditions, including GLP-1 therapy where appropriate, in a fully remote

Formación

• Active NP license in California.
• Board certification in Adult-Gerontology or geriatric-focused Family NP.
• Experience with Medicare documentation and E/M coding.

Responsabilidades

• Conduct initial longevity assessments and follow-up visits via telehealth.
• Perform cognitive screening and document results in the EHR.
• Prescribe and manage GLP-1 therapy for appropriate patients.
• Conduct advance care planning conversations and chronic disease management.
