Job summary

Total Life Inc. is seeking a Telehealth NP to perform initial longevity assessments and ongoing geriatrics-focused follow-ups for adults 65+. You will collaborate with our geriatric CMO and receive full training and clinical support.

Responsibilities include comprehensive geriatric assessment, cognitive screening, medication review, chronic disease management, and advance care planning, all conducted remotely with Medicare billing in mind.

Qualifications

• Active, unrestricted NP license in New Jersey.
• Board certification in geriatric-focused NP track.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.

Responsibilities

• Deliver initial longevity assessments and follow-ups via telehealth.
• Perform comprehensive geriatric assessment and cognitive screening.
• Review medications and manage chronic conditions.
• Conduct advance care planning conversations.
• Document in EHR to support Medicare billing.
