CareStar, Inc. is seeking a Clinical Assessor to join the Iowa Clinical Department serving Black Hawk County and surrounding areas.

You will conduct comprehensive clinical assessments, develop personalized care plans, and coordinate services for individuals to improve independence and quality of life. The role requires licensure as RN, LISW, LMHC, or LP; strong organizational and communication skills; and ability to travel within the region.

Qualifications

• Active professional license in RN, LISW, LMHC, or LP.
• Experience in assessment, care coordination or related settings preferred.
• Strong organization, communication and leadership abilities.
• Reliable transportation for regional travel and valid driver's license.

Responsibilities

• Conduct comprehensive clinical assessments and reassessments within required timelines.
• Develop personalized care plans and connect clients with needed services.
• Document findings accurately in required systems and maintain records.
• Communicate with members, caregivers, providers and staff as needed.
