Job summary

UnitedHealthcare is seeking a Transition Care Coordinator to apply clinical knowledge and coordinate member transitions across settings for predominantly medically complex individuals. You will work with the Care Coordination Team to ensure access to services, education, and cost-effective care.

You must reside within commutable distance to Wilson, Smith, and Davidson Counties and be able to travel up to 50% within the territory.

Qualifications

• Current Nursing License (RN) or LBSW License.
• 2+ years of clinical experience as an RN or LBSW.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Excellent organization, communication and time management skills.
• Reside in or within a commutable distance to Wilson, Smith, and Davidson Counties.
• Access to reliable transportation and valid US driver's license with travel up to 50%.

Responsibilities

• Apply clinical knowledge to effect efficient case management for Medicare/Medicaid benefits.
• Incorporate evidence-based clinical practices into care coordination.
• Deliver member-centered, individualized self-management support and education.
• Ensure appropriate utilization and consistent health plan benefits access.
• Conduct telephonic and face-to-face assessments per policy and timelines.
• Serve as member advocate and facilitator to remove care barriers.
• Coordinate community resource referrals to members and providers.
• Develop and implement member-centered care plans with families and providers.
• Collaborate with Care Coordination Team to improve quality and efficiency.
• Monitor hospital encounters and safety during transitions.
• Refer clinical decisions beyond authority to manager for review.
• Participate in Health Plan Quality Improvement processes.
