Job summary

Socket.dev is seeking a Nurse liaison to coordinate care and communicate medical information among Injured Worker, Insured, and Carrier to facilitate treatment and return to work. This remote, field-based role requires regular local travel up to 2 hours.

You will follow CM plans, accept referrals, record data in CaseAnyplace, and attend physician appointments to collect diagnosis, prognosis, and rehab timelines.

Qualifications

• Current, unrestricted RN license; licensed where field work is provided.
• Certification within 4 years preferred (CCM, CRC, CLNC, CRRN).
• Experience in case management for injured workers is preferred.

Responsibilities

• Coordinate care and communicate medical information among Injured Worker, Insured, and Carrier.
• Attend physician appointments and relay prognosis and treatment plan.
• Record data in CaseAnyplace and submit timely monthly reports.
• Travel regionally up to 2 hours and provide translation as needed.
