Job summary

Socket.dev seeks a Nurse liaison to coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to facilitate treatment, rehabilitation, and return to work. This remote, field-based role requires regular local travel up to 2 hours in your region.

You will follow CM plan policies, accept referrals, collect data, attend physician appointments, and promote teamwork while maintaining licensure and completing annual training.

Qualifications

• Registered Nurse with current, unrestricted license in each state where field case management is provided.
• Discipline eligibility to practice independently within scope.
• Education: nursing program and ongoing continuing education.
• Certification: CCM/CRC/CLNC/CRRN preferred within 4 years of hire.
• Experience: 2 years FTE direct case management for injured workers or 2 years under supervision preferred.

Responsibilities

• Follow CM plan policies and retain accountability for the CM process.
• Accept referrals and schedule initial assessments.
• Record data and billing; submit monthly reports.
• Attend physician appointments and document diagnoses, prognosis, and rehab plans.
• Coordinate transportation and translate as needed when required.
• Promote teamwork and maintain licensure/training.
