Job summary

Socket.dev is seeking a Nurse liaison to coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to support treatment and return to work. This remote, field-based role involves local travel up to 2 hours in your region.

The position requires an active RN license in relevant states, independent assessment eligibility, and 2 years of case management experience with ongoing CE. Certifications like CCM/CRC/CLNC/CRRN are preferred.

Qualifications

• Current, unrestricted state RN license.
• Discipline eligibility for independent assessment in applicable state.
• Completion of nursing program and ongoing CE as required.
• Certification within 4 years of hire (e.g., CCM, CRC, CLNC, CRRN) preferred.
• Two years FTE direct case management for injured workers or two years under supervision preferred.

Responsibilities

• Coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to facilitate treatment, rehabilitation, and return to work.
• Record data and billing in CaseAnyplace; submit timely monthly reports (Preliminary, Initial, Progress, Closing).
• Attend physician appointments; obtain diagnosis, prognosis, treatment plan, rehab length, estimated RTW, and MMI/Full Recovery as appropriate.
• Promote teamwork with all staff members and maximize accurate billable hours per monthly target (8 hrs/day).
• Provide translation as needed for bilingual nurses.
• Travel locally up to 2 hours; coordinate transportation as needed.
