Job summary

Cognizant is seeking an ER Medical Coder – E/M, Injections & Infusions to support healthcare clients remotely. You will review documentation, assign ICD-10-CM codes for ED and outpatient encounters, and ensure accuracy and regulatory compliance while collaborating with providers and coding teams.

The role requires certification, outpatient experience, and strong communication skills. This remote position offers flexible hours and a pathway to contribute to diverse projects within Cognizant.

Qualifications

• Certification required: CCS, RHIT, RHIA, CPC, or COC.
• Minimum of 2 years of facility-based outpatient coding experience.
• Experience coding Emergency Department encounters, including E/M coding.
• Experience with injections and infusion coding in outpatient or hospital settings.
• Knowledge of federal, state, and payer regulations related to medical coding.
• Ability to interpret medical records, reports, and chart documentation accurately.
• Strong analytical, judgment, and decision-making skills.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Review clinical documentation and assign ICD-10-CM diagnosis and procedure codes for outpatient settings (ED, observation, outpatient surgery, diagnostic encounters).
• Utilize encoder applications to assign codes per company policies and regulatory guidelines.
• Query physicians to obtain or clarify diagnoses and procedures through compliant processes.
• Perform peer reviews of coding auditors and coders to support quality goals.
• Prepare and complete coding-related reports as requested.
• Maintain a minimum coding accuracy rate of 98% while meeting productivity standards.
• Ensure compliance with federal, state, payer, and client-specific coding guidelines and regulations.
• Work effectively in a remote environment while managing multiple systems and priorities.
