Qualifications

• Bachelor’s degree preferred; can be substituted with 1+ years of related experience.
• Minimum of one year of experience serving individuals with disabilities and aging adults preferred.
• Remote with regular business travel; must reside in Tampa Bay or Jacksonville, FL.
• Strong customer service and MS Office proficiency.
• Excellent verbal and written communication; ability to build relationships with stakeholders.

Responsibilities

• Enrollment & Service Coordination: follow referrals and complete enrollment docs.
• Identify barriers and triage to timely enrollment.
• Coordinate with case management/providers to minimize enrollment cycle times.
• Ensure first payment to providers is received on time.
• Document enrollment status in portal and systems.
• Assess participant’s ability to communicate and participate in self-directed models.
• Educate participants on employer responsibilities and payroll processes.
• Provide train-the-trainer instruction on program rules and Financial Management Systems.
