You will connect with eligible patients, educate and enroll them into preventive care programs, and coordinate with teams to support ongoing patient care. Strong customer service and communication skills are essential.

Formación

• High school diploma or equivalent.
• Healthcare experience preferred.
• Bilingual in Spanish preferred.
