Job summary

Form Health, a virtual obesity medicine clinic, is seeking an Endocrinologist with obesity medicine expertise to join our telemedicine care team.

The role is a fully remote W-2 position (US-based) with a base salary of $210,000–$250,000 USD. You will lead diabetes and obesity management, collaborate with APPs and Dietitians, and deliver evidence-based care across diverse patient populations.

Qualifications

• Must have an MD or DO degree.
• Board certification in Endocrinology by ABIM is required.
• ABOM certification preferred.
• 2 years of clinical experience in obesity medicine or endocrinology, or completed an endocrinology/obesity fellowship.
• Active/unrestricted medical license in any US state and eligibility for others.

Responsibilities

• Provide direct telemedicine care focusing on obesity and diabetes management.
• Work with Endocrine APPs and Registered Dietitians.
• Review patient charts and ensure quality and continuity of care.
• Provide advanced diabetes management including CGMs and pumps.
• Mentor and collaborate with APPs and trainees in endocrinology.
