Job summary

Form Health is a virtual obesity medicine clinic delivering multidisciplinary evidence-based treatment through telemedicine. We seek a Physician specialized in Endocrinology and Obesity Medicine to join our physician-led care team.

This US-based ABIM-certified Endocrinologist role requires MD/DO, active license, and eligibility for additional state licenses. Remote W-2, 32–40 hrs/week, with 80% clinical care and 20% administrative time, offering competitive compensation and full benefits.

Qualifications

• MD or DO degree from an accredited medical school.
• Valid ABIM board certification in Endocrinology.
• ABOM certification is highly preferred.
• 2+ years of clinical experience or fellowship in endocrinology or obesity medicine.
• Active unrestricted medical license in any US state; licensure across states if needed.
• Must be enrolled with Medicare.
• Experience managing patients with CKD/ESRD and CAD/CV conditions.
• Expertise in CGMs and insulin therapy; telemedicine experience preferred.
• Bilingual Spanish (desirable but not required).

Responsibilities

• Provide direct patient care in telemedicine for obesity and diabetes.
• Collaborate with APPs and dietitians for diabetes treatment.
• Review charts and conduct visits to ensure quality and continuity.
• Develop advanced treatment plans for complex endocrinology cases.
• Prescribe non-controlled medications for obesity and diabetes.
• Ensure adherence to clinical protocols and documentation.
• Support professional development and continuing education within the team.
• Participate in team-based care and quality improvement initiatives.
• Attend departmental meetings and contribute to program enhancement.
