Job summary

Form Health is a virtual obesity medicine clinic hiring physicians specialized in Endocrinology and Obesity Medicine to join a multidisciplinary care team. A valid US medical license and ABIM Endocrinology board certification are required, with ABOM preferred.

The role focuses on diabetes management, CGMs, insulin pumps, and collaborative care with APPs and RDs. This fully remote W-2 position offers flexible scheduling: 32, 36, or 40 hours per week, with 80% clinical work and 20% administrative

Qualifications

• MD or DO degree is required.
• Active ABIM certification in Endocrinology is required.
• ABOM certification is highly preferred.
• Active and unrestricted medical license in any US state, with eligibility for others.
• Fellowship in Endocrinology or Obesity Medicine may substitute years of experience.

Responsibilities

• Provide direct patient care in telemedicine for obesity and diabetes management.
• Collaborate with Endocrine APPs and RDs in treatment planning.
• Review charts and manage CKD/ESRD and CAD/CV conditions.
• Support advanced diabetes management using CGMs and insulin pumps.
• Prescribe non-controlled medications for obesity and diabetes.
• Ensure consistent clinical documentation and treatment pathways.
