Job summary
Form Health is seeking an ABIM-certified Endocrinologist with obesity medicine expertise to join our fully remote care team. You will provide evidence-based obesity and diabetes management through telemedicine and collaborate with APPs and registered dietitians to deliver high-quality patient care.
The role focuses on complex comorbidity management (CKD/ESRD, CAD/CV), integrating CGMs and insulin pumps, with 32–40 hours weekly.
Qualifications
- MD or DO degree required
- ABIM Endocrinology board certification required
- Active US medical license with eligibility for additional states
- Preferred ABOM certification
- 2 years clinical experience in obesity medicine or endocrinology or completed fellowship
Responsibilities
- Provide direct patient care via telemedicine focusing on obesity and diabetes management
- Collaborate with Endocrine APPs and Registered Dietitians
- Review charts and manage CKD/ESRD and CAD/CV conditions
- Advance treatment planning for complex endocrinology cases
- Prescribe non-controlled medications for obesity and diabetes
- Ensure documentation and adherence to care pathways
- Mentor colleagues and support continuing education
- Participate in team-based care and quality improvement
- Attend departmental meetings and contribute to program improvements
