Job summary

Form Health is a US-based remote obesity medicine clinic delivering multidisciplinary care via telemedicine. We seek an ABIM-certified Endocrinologist with obesity medicine focus to lead and collaborate with APPs and dietitians to manage patients with Type 2 diabetes and obesity.

The role emphasizes safe management of complex comorbidities, flexible scheduling (32/36/40 hours), and a commitment to evidence-based care. Base pay ranges from $210k to $250k with full benefits.

Qualifications

• MD or DO degree required.
• ABIM board certification required.
• Active, unrestricted medical license in any US state.
• ABOM certification highly preferred.
• 2 years of clinical experience or completed fellowship in endocrinology/obesity medicine.
• Currently opted-in with Medicare.
• Experience with telemedicine and mentoring APPs/Dietitians.
• Willing to work 32/36/40 hours per week.

Responsibilities

• Provide direct patient care in telemedicine, focusing on obesity and diabetes management.
• Collaborate with Endocrine APPs and Registered Dietitians on treatment plans.
• Review charts and conduct visits to manage CKD/ESRD and CAD/CV conditions.
• Prescribe non-controlled meds for obesity and diabetes in coordination with APPs.
• Ensure documentation and treatment pathways follow established guidelines.
• Support professional development and participate in quality improvement.
• Attend departmental meetings and contribute to diabetes/obesity programs.
