Job summary

Form Health is seeking an ABIM-boarded Endocrinologist specialized in obesity medicine for a fully remote US-based role. You will provide evidence-based telemedicine care with endocrinology expertise, collaborating with APPs and dietitians to manage complex diabetes and obesity cases.

We offer flexible W-2 roles of 32, 36, or 40 hours per week, with strong patient‑facing care focus and opportunities to lead clinical initiatives.

Qualifications

• MD or DO degree and ABIM Endocrinology board certification.
• Board certification in obesity medicine preferred (ABOM).
• Active US medical licensure and eligibility for additional licenses.

Responsibilities

• Provide direct telemedicine care for obesity and diabetes management in adults.
• Collaborate with APPs and dietitians for multidisciplinary care.
• Manage CKD/ESRD and cardiovascular comorbidity in complex patients.
• Utilize CGMs and insulin pumps within treatment plans.
• Contribute to education, mentoring, and clinical program development.
