Job summary

Form Health, a virtual obesity medicine clinic, seeks an ABIM-boarded Endocrinologist to join a multidisciplinary chronic disease team. This fully remote W-2 role offers flexible hours (32/36/40 per week) with 80% direct patient care and 20% administration, requiring an active US medical license and ABIM certification.

Ideal candidates will have expertise in obesity and diabetes management, experience with CGMs and insulin pumps, and a collaborative approach with APPs and RDNs.

Qualifications

• MD or DO degree with ABIM Endocrinology certification.
• ABOM certification preferred.
• 2 years of clinical obesity or endocrinology experience or accredited fellowship.
• Active, unrestricted US state license; multi-state eligibility.

Responsibilities

• Provide direct telemedicine care for obesity and diabetes management.
• Collaborate with APPs and RDNs in diabetes care.
• Manage complex comorbidities (CKD, ESRD, CAD/CV) and CGM/insulin therapy.
• Document care plans and follow evidence-based protocols.
• Mentor and support APPs and residents; contribute to quality initiatives.
