Job summary

Form Health, a remote obesity medicine clinic, is seeking an ABIM-certified Endocrinologist to join a multidisciplinary team. The role focuses on obesity and diabetes management in a telemedicine setting, collaborating with APPs and RDs. A valid US license and ABIM certification are required, with ABOM preferred.

The position is fully remote, W2, part-time (minimum 20 hours/week: 16 clinical, 4 admin), and supports physician-led care with a patient-centered approach.

Qualifications

• MD/DO degree with ABIM Endocrinology board certification.
• Active medical license in any US state; ability to obtain additional state licenses.
• 2 years clinical obesity or endocrinology pharmacotherapy experience, or completion of an endocrinology/obesity fellowship.
• Excellent communication and teamwork; telemedicine experience preferred.

Responsibilities

• Provide direct patient care via telemedicine for obesity and diabetes.
• Collaborate with APPs and RDs in diabetes treatment.
• Review charts; conduct follow-up visits for CKD/ESRD and CAD/CV.
• Develop advanced endocrinology treatment plans.
• Prescribe non-controlled medications for obesity and diabetes.
• Ensure documentation and treatment pathways are followed.
• Mentor colleagues and support education in endocrinology and obesity.
• Participate in team-based care and quality improvement.
• Attend departmental meetings and contribute to program quality.
