Job summary

Form Health Inc. is seeking ABIM-endorsed Endocrinologists to join its telemedicine-based obesity medicine program.

You will provide evidence-based care for obesity and diabetes, collaborate with APPs and RDs, and support patients with CKD/ESRD and cardiovascular conditions. This fully remote W-2 role offers flexible hours: 32, 36, or 40 per week, with 80% patient-facing and 20% administrative time, plus comprehensive benefits, 401k, paid parental leave, and access to Form4Form weight loss

Qualifications

• MD or DO degree required with ABIM Endocrinology board certification.
• ABOM certification highly preferred.
• Active US medical license in any state with eligibility for additional licenses.
• 2+ years in obesity medicine or endocrinology, or fellowship in endocrinology/obesity medicine.
• Must be able to practice in telemedicine with guidance from ABIM/ABOM requirements.

Responsibilities

• Provide direct telemedicine care focusing on obesity treatment and diabetes management.
• Collaborate with APPs and Registered Dietitians to coordinate patient care.
• Review charts and visits to manage CKD/ESRD and CAD/CV conditions.
• Prescribe non-controlled medications for obesity and diabetes with coordination.
• Ensure documentation and treatment pathways follow guidelines.
• Mentor and support clinical education for colleagues.
• Participate in team quality improvement and departmental meetings.
