Job summary

One Community Health is seeking an Empanelment Analyst to support the provider empanelment program by maintaining patient-provider assignments, monitoring panel performance, and developing reports and dashboards to drive operational decisions.

Based in Midtown - Sacramento, CA with a hybrid schedule requiring 3 days on-site, this role collaborates with clinic managers, care teams, and leadership to optimize panel size, provider capacity, and data quality.

Qualifications

• Diploma or related field or equivalent experience.
• Two years of experience in healthcare operations, analytics, panel management, population health, or related field.
• Intermediate advanced proficiency in Microsoft Excel.
• Experience with healthcare data, reports, and operational metrics.
• Strong analytical, organizational, and problem-solving skills.

Responsibilities

• Generate monthly panel reports and review PCP assignments.
• Calculate continuity of care percentages and adjust for coverage schedules.
• Resolve unassigned patients by reviewing appointment history and records.
• Monitor influx of new patients and analyze provider capacity for absorption.
• Track panel size against right size and discuss adjustments with leadership.
• Maintain and update patient status changes and PCP assignments in the PM system.
• Collaborate with clinic managers, providers, and care teams to maintain empanelment processes.
• Develop workflows and dashboards for panel management, empanelment, and risk stratification.
