Job summary

Bluestone Physician Services seeks a Float Care Coordinator to manage DHS and CMS activities for Minnesota Senior Health Options and related programs. You will travel locally to serve community members, while working from home with regular daytime hours.

The role emphasizes care coordination, assessments, and collaboration with diverse care teams to ensure high-quality outcomes for seniors and disabled individuals.

Qualifications

• Minnesota RN license or MN Licensed Social Worker credential.
• 1+ year geriatric/care coordination experience preferred.
• Must have a valid driver’s license.

Responsibilities

• Manage member caseloads within geographic area.
• Coordinate face-to-face visits and Elderly Waiver activities.
• Conduct PCA assessments and living tools for Assisted Living.
• Review health needs, set goals, and develop care plans.
• Connect members with community and state resources.
• Complete required documentation and collaborate with care teams.
• Support transitions of care and safe discharge planning.
