Qualifications

• RN Compact license required.
• Minimum 5+ years clinical experience with Medicare members.
• Minimum 2+ years discharge planning/home health care coordination.
• Preferred bilingual ability.
• Certified Case Manager or national certifications preferred.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.
• Ability to work independently.
• Excellent analytical and problem-solving skills.
• Strong communication, organizational, and interpersonal skills.
