Job summary

Qualifications

• A minimum of 1 year of recent primary care experience as a Nurse Practitioner in family medicine, primary care, or urgent care.
• Knowledge of quality measurement in chronic disease management.
• Self-motivated with initiative and problem solving; advocates for patients.
• Strong verbal, written, and electronic communication skills.
• Organized with ability to multi-task and manage competing priorities.
• Adaptable and flexible; able to work in a clinical team setting remotely.
• Ability to work without direct supervision and practice autonomously.
• Proficiency with information management and technology.
• Willingness to obtain multi-state licensures.

Responsibilities

• Provide primary and family-based evidence-based care including assessment, diagnosis, treatment, and disposition planning.
• Handle administrative duties: opening/closing clinic, inventory, patient follow-up, insurance verification, payments, and restroom/clinic upkeep.
• Manage patient appointments and perform routine examinations to support positive outcomes.
• Discuss healthy lifestyle choices with patients and coordinate with other providers for comprehensive care.
• Encourage patient engagement in health screenings, behavioral health, wellness, and chronic condition management.
