Qualifications

• Minimum 2 years in healthcare administration at managerial level.

Responsibilities

• Consolidates inbound referral processes and reports data to senior management.
• Serves as single point of contact to verify insurance, assess needs, and respond to referrals.
• Engages with referrals to ensure appropriate care transitions.
• Assists patients and families in identifying services and community supports.
• Collects data to support admissions and signs forms.
• Reviews data systems to ensure MIS data integrity.
• May temporarily assume additional duties or fill in during vacancies.
• Other duties as assigned.
