Job summary

OPCO Skilled Management is seeking a Central Intake Nurse for a remote, full-time role. You will collect referral documentation from physicians, insurers, and providers to determine service appropriateness and coordinate patient care plans with providers.

The position requires a state nursing license and at least two years in healthcare administration at a managerial level, with responsibility for intake processes, data integrity, and liaison with hospitals and facilities.

Qualifications

• High school diploma required.
• Current nursing license for appropriate state.
• Two years of healthcare administration managerial experience.

Responsibilities

• Consolidates inbound referral processes, records and reports data related to admissions, discharges, and admission inquiries to senior and facility management.
• Provides a single point of contact to verify insurance coverage, assess care needs, determine facility availability, and respond to referrals from hospitals in a succinct, organized fashion.
• Receives, updates, and engages with referrals, ensuring the organization provides appropriate level of care, while soliciting additional information for smooth transitions.
• Assists patients and families by determining services and support best suited for each patient and identifying community resources.
• Collects data to facilitate admissions; ensures forms are signed and filled, establishing a single access-point for patients and organizations.
• Reviews and audits the MIS data elements to ensure data integrity input into CRM, IRM, PCC systems.
• May temporarily assume additional duties or a vacancy; other duties may be assigned as needed.
