Job summary

Pinnacle Services is seeking a Case Manager for the Scott County team to work full-time after an initial in-office period. You will meet clients in person, assess needs, and coordinate resources while maintaining independence in case management tasks.

The role involves traveling for client meetings in Scott County, collaborating with interdisciplinary teams, and ensuring services meet client needs; licensure as an SW is preferred and a valid driver’s license is required.

Qualifications

• Bachelor's in social work or related field required or equivalent.
• One year of full-time experience as a social worker / case manager.
• Experience with developmentally disabled or related conditions preferred.
• Valid driver's license with insurance.
• Willingness to commute to Hennepin and Scott Counties for meetings.
• Background checks required.

Responsibilities

• Drive to client meetings in Scott County (average ~4 per week).
• Assess areas where assistance is needed and coordinate resources.
• Monitor service delivery and work with providers to meet needs.
• Develop service plans and attend interdisciplinary team meetings.
• Perform case-noting to create billable units.
