Job summary

Pinnacle Services is seeking a dedicated Case Manager for our Scott County team. The role involves meeting clients in person, coordinating services, and developing individualized service plans in collaboration with interdisciplinary teams.

You will assess needs across housing, transportation, health, and supports, driving community-based meetings and documentation. A bachelor's in social work or related field is required, with licensure pursued or held.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field.
• Bachelor's degree in any field with related coursework acceptable.
• One year of full-time experience as a social worker/case manager/care coordinator.
• One year of experience in education or treatment of persons with developmental disabilities or related conditions.
• At least one course focused on developmental disabilities; transcript required.
• Valid driver's license and acceptable driving record; vehicle with current insurance.
• Willingness to commute to areas of Hennepin and Scott Counties for meetings.

Responsibilities

• Drive to attend client meetings in Scott County, ~4 meetings per week.
• Assess areas where assistance is needed.
• Offer and coordinate resources for individuals being supported.
• Monitor service delivery and work with providers to meet needs.
• Develop service plans and attend interdisciplinary team meetings.

Case Managers have a lot of independence while managing their caseloads, therefore someone who would prefer group work to be self-led, consulting and collaborating with others, then returning to their independent tasks would succeed in this role.

Job Responsibilities and Duties

Case Managers have the opportunity to work with individuals that may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging. For our Scott County position, Case Managers have the opportunity to work full-time after 60 days in office, with the expectation of meeting with clients in person.

The Case Manager is responsible for:

• Driving to attend client meetings in Scott County, with an average of 4 client meetings per week.
• Assessing the areas of the individual's life where assistance is needed.
• Offering and coordinating resources for the individuals being supported.
• Monitoring service delivery and working with providers to ensure the individual's needs are met.
• Service plan development
• Attend regular in-person meetings with individuals served and their interdisciplinary teams.
• Case Noting client-related work to create Billable Units

Salary

Non-LSW wage: $49,000/year salaried for 40hr/week.

LSW wage: $51,600/year salaried for 40hrs/week.

Requirements

Required Qualifications

• Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field

or

• Be a graduate from an accredited four-year college with a major in any field

and

• One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency.

or

• One year of experience in education or treatment of persons served with developmental disabilities or related conditions

or

• At least one course focused on developmental disabilities is required. Candidates must provide an official college or university transcript to verify completion of this coursework.

• Applicants must have a valid driver's license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance.
• Applicants must successfully clear local and/or stated background checks.
• Applicants must be willing to commute to areas of Hennepin and Scott Counties for potential meetings.

Preferred Qualifications

• One year of full-time experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
• Possess a LSW or higher form of BOSW Licensure.
