Job summary

Cognizant is seeking an Emergency Department Coder to ensure accuracy, quality, and compliance of medical coding across outpatient settings. You will review clinical documentation, assign ICD-10-CM and CPT/HCPCS codes, and collaborate with physicians to resolve coding queries.

This fully remote role requires CPC or CCS certification, strong knowledge of coding guidelines, and a proven ability to maintain at least 98% accuracy while meeting productivity expectations.

Qualifications

• Certified Professional Coder (CPC) or Certified Coding Specialist (CCS) or equivalent.
• Experience coding and auditing outpatient, observation, ED and outpatient encounters.
• Strong knowledge of ICD-10-CM, CPT, HCPCS and regulatory guidelines.
• Experience with electronic encoder applications and coding systems.
• Ability to interpret clinical documents and clarify codes with physicians.
• Proven track record of meeting productivity and at least 98% accuracy.

Responsibilities

• Review clinical documentation and assign diagnosis and procedure codes for outpatient settings.
• Query physicians to obtain clarification on diagnoses and procedures.
• Use encoder tools to assign codes per policies and guidelines.
• Perform peer reviews and quality audits to ensure accuracy and compliance.
• Prepare and deliver coding audit reports and actionable recommendations.
• Maintain a minimum coding accuracy rate of 98% while meeting productivity standards.
