Job summary

Optima Medical Management Group is seeking an RN Clinical Case Manager for the Enhanced Care Management (ECM) program. This is a non-bedside, RN-led role serving as the clinical authority for ECM care planning and regulatory compliance.

The RN collaborates with ECM Lead Care Managers to coordinate care, identify risks, and ensure medical appropriateness across transitions and outcomes. Strong clinical judgment and communication are essential.

Qualifications

• Active California & GA RN license required.
• ADN required; BSN preferred.
• Minimum 3 years clinical nursing experience.
• Experience with Medi-Cal and/or managed care populations.
• Strong clinical judgment and communication skills.

Responsibilities

• Conduct clinical assessments based on history, diagnoses, utilization patterns, meds, and risk indicators.
• Develop, review, and clinically approve individualized ECM care plans.
• Identify and monitor clinical and utilization risks impacting outcomes.
• Provide RN oversight to ECM Lead Care Managers.
• Review medication lists for safety and adherence.
