Job summary

Full Circle Health Network is seeking a Network Outreach Coordinator to coordinate referrals for Enhanced Care Management. This remote, part-time role offers a path to full-time and a benefits package after transition. You will work with managed care plans and providers to ensure clients receive appropriate, whole-person care.

You will engage mothers, children, and families, maintain required documentation, and participate in ongoing HIPAA training and community outreach initiatives.

Qualifications

• At least 3 years of relevant work experience.
• High school diploma or GED required.
• Long-time community resident with knowledge of local resources.
• Ability to work flexible hours including evenings/weekends.

Responsibilities

• Conduct outreach and engagement to facilitate ECM enrollment.
• Serve as day-to-day contact for managed care plans and network providers.
• Develop understanding of the Full Circle provider network to support referrals.
• Monitor client assignments and complete tasks within required timeframes.
• Engage clients via text, email, telephone, and in-person.
• Escalate issues to the Network Operations Director and support quality improvement.
• Complete documentation in the required electronic system within timeframes.
• Attend meetings with managed care plans and providers and trainings.
