Job summary

Humana is seeking a Director, Health Solutions – Chronic Condition Management to lead strategy and performance for a national portfolio of Medicaid products and programs. You will translate population health insights into opportunities, set investment priorities, and scale solutions across states.

You will build a high-performing team, own roadmaps, and influence enterprise decisions while collaborating across clinical, operational, analytical, and product domains.

Qualifications

• Bachelor’s degree.
• 7+ years of experience in healthcare strategy or population health strategy.
• 5+ years of scaling healthcare products or population health solutions.
• 3+ years of formal people-leadership experience.

Responsibilities

• Lead the development and execution of the national Chronic Condition Management product strategy and roadmap.
• Translate insights, needs, and evidence into actionable product opportunities.
• Establish portfolio vision, priorities, and investment strategy aligned with Medicaid goals.
• Develop business cases with opportunity, outcomes, value proposition, and requirements.
• Prioritize investments based on impact, affordability, quality, scalability, and readiness.
• Ensure national-scale solutions with state adaptations.
• Set portfolio metrics, monitor outcomes, and drive improvements.
• Support Medicaid RFPs and growth initiatives with design and content contributions.
• Build and lead a high-performing team with healthcare strategy and portfolio development expertise.
