Job summary

Humana is seeking a Director, Population Health Strategy – Chronic Condition Management to own the national portfolio of products, programs, benefits, partnerships, and care models for Medicaid members with chronic conditions. You will lead a team using population data, clinical evidence, and market priorities to shape strategy, measure performance, and scale solutions.

Qualifications

• Seven+ years in healthcare strategy or population health translating needs into strategies.
• Five+ years scaling healthcare products or population health solutions.
• Three+ years of formal people leadership experience.
• Experience using clinical evidence and population data to inform design and performance decisions.
• Experience developing multi-year roadmaps, business cases, and investment recommendations.
• Experience presenting to senior executives or governance bodies.
• Strong cross-functional collaboration across clinical, operational, analytical, financial, digital, product, or market teams.

Responsibilities

• Lead the national Chronic Condition Management strategy and multi-year roadmap.
• Translate insights into actionable product opportunities.
• Set portfolio vision and investment strategy aligned with Medicaid and enterprise goals.
• Develop business cases detailing opportunity, outcomes, value, and resources.
• Prioritize investments by member impact, clinical opportunity, and value.
• Ensure scalable solutions with state adaptations.
• Establish portfolio performance metrics and drive continuous improvement.
• Support Medicaid RFPs and growth initiatives.
• Build, develop, and lead a high-performing strategy team with healthcare expertise.
