Job summary

Baystate Health seeks a Care Team Navigator to act as the primary contact between patients with dementia, their caregivers, and the Baystate Memory Assessment and Care Center (MACC). Much work is via video or telephone, and extensive training in ethics, aging, dementia care, and care planning is provided.

The role emphasizes excellent communication and may prefer bilingual English/Spanish speakers. No college degree is required, and compensation ranges are publicly posted for base salaries

Qualifications

• Excellent communication skills are required.
• Fluency in Spanish is desired but not required.
• Role does not require a college degree or health care background.

Responsibilities

• Provide dementia-specific education and support to patient and family.
• Screen dyads for unmet care needs including clinical and medication issues.
• Guide dyads through advanced care planning with MACC clinical team.
• Connect dyads to local community services, including respite care.
• Collect information about medications and assist in reconciliation and adherence.
• Assess medication side effects and changes in function and behavior with the care team.
